More than 30 years ago we saw that the needs of San Francisco's frail elderly were not being met, so we formed On Lok Senior Health Services to provide them with comprehensive, integrated health and long-term care. We combined medical, social, nutritional, rehabilitation, transportation and support services--making it possible for these vulnerable seniors to continue to live in their homes. Our success in San Francisco led to the national replication of our program as PACE (Program of All-inclusive Care for the Elderly). To launch this replication we helped the first ten sites start their programs.
