PACE, or Program of All-inclusive Care for the Elderly, is a unique and comprehensive health care provider, integrating medical, social, nutritional, rehabilitation and support services. Because of PACE's key features, frail seniors live with dignity and independence in their own homes for a long as possible, receiving the best in senior health care.

Members
We call our members “participants.” PACE participants are seniors with multiple health problems:- They are at least 55 years old
- They live in a defined service area
- They require a nursing home level of care
- They are capable of living in the community without jeopardizing their health or safety at the time they enroll.
Physical Setting
PACE participants receive most of their health care at a community center licensed as an adult day health care program and a primary care clinic. Designed to be inviting, comfortable and safe, the centers have separate areas for dining, activities, rehabilitation, primary care and personal care.The number of days a senior attends the center depends upon individual need. Specially equipped vans bring participants to and from the center each day.
